PS Nurse Practitioner - HMHI HOME Program

Job Summary

The H.O.M.E. Program is looking for a Nurse Practitioner to provide primary care to people with disabilities. This individual will work with an interdisciplinary team that includes primary care providers, psychiatrists, therapists, occupational therapists, speech therapists, dietician and behavior analysts. This position will provide primary, medical care to pediatric patients ranging in age from 4-22.
